Read More Background: Postoperative Nausea and Vomiting (PONV) is defined as any nausea, retching or vomiting occurring during the first 24-48 hours after surgery with a reported incidence of 30% in all post-surgical patients and up to 80% in high risk groups especially in laparoscopic surgeries under general anaesthesia. Many drugs have been tried and tested for abolishing PONV. We evaluated the efficacy of low dose ketamine in decreasing the incidence and severity of PONV following elective laparoscopic surgeries under general anaesthesia.Method: 40 patients belonging to American Society of Anaesthesiologists Physical status I who were scheduled for elective laparoscopic surgeries under general anaesthesia were recruited and randomized into two groups Group A and Group B. Standard protocol for general anaesthesia were followed for all patients. Patients in Group A received Intravenous ketamine of 0.2mg/kg and Patients in Group B received the saline control half an hour before extubation. The incidence and severity of PONV, number of rescue drug administration, and side effects were recorded for first 24 postoperative hours in different time periods.Results: Demographic profile were comparable among the groups. The incidence and severity of postoperative nausea were significantly higher in Group B when compared to Group A for the first 6 postoperative hours (p<0.05) and thereafter, it was comparable between the groups. The incidence of postoperative vomiting was significantly higher in Group B when compared to Group A for the first 2 postoperative hours (p<0.04), and then it was comparable between the groups. The number of rescue medications were significantly higher in Group B when compared with Group A. No complications were noted.Conclusion: Low dose intravenous ketamine (0.2 mg/kg) administered as a single bolus reduced the incidence and severity of postoperative nausea and vomiting in adults undergoing laparoscopic surgeries under general anaesthesia without any complications.

Read More Background: Aerobic exercise is recommended starting from the subacute phase of low back pain, which can increase daily activities. This study aims to analyse the effect of adding walking aerobic exercise on pain intensity and disability status of mechanical chronic low back pain in office workers at Dr. Soetomo Hospital.Methods: True experimental with pre-test and post-test randomized control group design. Each group consisted of 16 respondents. The treatment group received walking aerobic exercise therapy three times a week for 30 minutes with gradually increasing intensity and conventional low back pain exercises for eight weeks. The control group only received conventional exercise therapy 3x/week for eight weeks. The parameters evaluated in this study were pain intensity values using NPRS and disability status using ODI. Measurements were made twice, namely before and after eight weeks of intervention.Results: There was a significant improvement in the NPRS score in the treatment group and the control group (p<0.000 and p<0.001), but there was no difference between the two groups after the intervention (p=1.000), and there was no difference (delta) in the NPRS score between groups (p=0.714). Read More Background: In comparison to the general adult female population, women afflicted by schizophrenia exhibit higher rates of unintended and undesired pregnancies. Maternal individuals grappling with severe mental health challenges face an increased susceptibility to postpartum psychosis, depression, and anxiety stemming from the presence of a child. Psychotic conditions often lead to the rejection of pregnancy, subsequently impinging upon maternal parenting capabilities. The predominant source of support for women with schizophrenia emanates from their familial networks.Case illustration: This paper presents the case of a 41-year-old adult female diagnosed with paranoid schizophrenia that was 40 weeks pregnant and admitted for medical care. The patient received treatment for restlessness and speech slurring. Comprehensive evaluations were conducted by healthcare professionals from the obstetrics and gynecology as well as psychiatry departments, leading to a decision to proceed with a cesarean section to terminate the pregnancy.Methods: Patients receive medical interventions and psychological assistance to stabilize their mental health conditions and avert relapses. Supportive psychotherapy and family psychoeducation, coupled with a collaborative approach involving discussions with obstetrician colleagues regarding childcare plans and supplementary psychiatric treatment, are implemented to enhance maternal well-being while attending to infant care.Conclusion: The obstetric team extensively deliberated the advantages and disadvantages of terminating the pregnancy, considering the provision of sustained care for both the patient and her unborn child. Given that the pregnancy termination was facilitated via cesarean section due to the patient's mental health concerns and the potential adverse effects of antipsychotic medication, the patient's family plays a pivotal role in providing assistance for infant care. Consequently, breastfeeding is contraindicated concerning the patient's mental health status and the potential consequences of antipsychotic treatment.

Read More Fetal tachyarrhythmia is a rare condition associated with a high risk of fetoplacental circulatory failure, fetal hydrops, and intrauterine fetal death. This report, present a case of fetal tachyarrhythmia that was successfully managed with a multidisciplinary approach in a tertiary hospital, but with prolonged prenatal resolution. A 36-year-old multigravida was diagnosed with fetal tachyarrhythmia at 30 weeks of gestational age. No secondary cause of fetal tachyarrhythmia was identified. A basic ultrasound examination revealed a normal heart structure without hydrops or polyhydramnios. The fetal heart rate was consistently at 230 bpm during the examination. M-mode echocardiography presented the 1:1 atrioventricular contraction with short intervals, confirming the diagnosis of sustained supraventricular tachycardia. Transplacental treatment was initiated using digoxin at a dose of 0.5 mg, followed by 0.25 mg intravenously every 8 hours. On the fifth day, the arrhythmia was not improved, thus oral combination therapy commenced with digoxin at a dose of 0.25 mg every 12 hours, along with propranolol at a dose of 40-20-20 mg, and this treatment was continued for the subsequent 5 weeks. Fetal heart rate, movement, and treatment toxicity were monitored daily, while fetal well-being was assessed on a weekly basis. Prenatal resolution was achieved 6 weeks after the treatment initiation. Digoxin levels reached a therapeutic concentration (1.75 ng/mL) without any signs of intoxication. A cesarean section was performed at 38/39 weeks of gestational age. The postnatal evaluation did not reveal any recurrence of tachyarrhythmia or neurodevelopmental disorders. Evaluation of atrioventricular contractions using M-mode echocardiography proved to be a straightforward method. In this case of sustained supraventricular tachycardia, the absence of fetal hydrops was likely attributed to ventricular contractions remaining below 230 beats per minute. The use of combination therapy demonstrated superiority over monotherapy in achieving prenatal resolution. Transplacental anti-arrhythmic combination therapy can be considered for cases of refractory tachyarrhythmia. Further large-scale population studies are necessary to determine appropriate therapeutic doses and adjust available drug options to achieve faster prenatal resolution of fetal tachyarrhythmias.
