Read More Fetal tachyarrhythmia is a rare condition associated with a high risk of fetoplacental circulatory failure, fetal hydrops, and intrauterine fetal death. This report, present a case of fetal tachyarrhythmia that was successfully managed with a multidisciplinary approach in a tertiary hospital, but with prolonged prenatal resolution. A 36-year-old multigravida was diagnosed with fetal tachyarrhythmia at 30 weeks of gestational age. No secondary cause of fetal tachyarrhythmia was identified. A basic ultrasound examination revealed a normal heart structure without hydrops or polyhydramnios. The fetal heart rate was consistently at 230 bpm during the examination. M-mode echocardiography presented the 1:1 atrioventricular contraction with short intervals, confirming the diagnosis of sustained supraventricular tachycardia. Transplacental treatment was initiated using digoxin at a dose of 0.5 mg, followed by 0.25 mg intravenously every 8 hours. On the fifth day, the arrhythmia was not improved, thus oral combination therapy commenced with digoxin at a dose of 0.25 mg every 12 hours, along with propranolol at a dose of 40-20-20 mg, and this treatment was continued for the subsequent 5 weeks. Fetal heart rate, movement, and treatment toxicity were monitored daily, while fetal well-being was assessed on a weekly basis. Prenatal resolution was achieved 6 weeks after the treatment initiation. Digoxin levels reached a therapeutic concentration (1.75 ng/mL) without any signs of intoxication. A cesarean section was performed at 38/39 weeks of gestational age. The postnatal evaluation did not reveal any recurrence of tachyarrhythmia or neurodevelopmental disorders. Evaluation of atrioventricular contractions using M-mode echocardiography proved to be a straightforward method. In this case of sustained supraventricular tachycardia, the absence of fetal hydrops was likely attributed to ventricular contractions remaining below 230 beats per minute. The use of combination therapy demonstrated superiority over monotherapy in achieving prenatal resolution. Transplacental anti-arrhythmic combination therapy can be considered for cases of refractory tachyarrhythmia. Further large-scale population studies are necessary to determine appropriate therapeutic doses and adjust available drug options to achieve faster prenatal resolution of fetal tachyarrhythmias.

Read More The aim of the study was to assess the influence of Sacubitril/Valsartan (S/V) on hemodynamic indices and heart remodeling events in rats with DOCA-salt arterial hypertension (AH). Experiments were performed on Wistar rats. Animals were randomly divided into the four groups: (I) Control (C) group, (II) DOCA-salt group, (III) S/V + DOCA -salt as drinking water, and (IV) S/V after development of DOCA-salt hypertension. Heart beat (HB)/min and systolic and diastolic blood pressure (SBP, DBP, mmHg) were obtained by tail-cuff method in the morning hours. The level of ET-1, IL-1, TNF-alpha, and NF-kB transcription factor were measured in the blood plasma of rats by ELISA kits method. Morphometric and histomorphological changes in heart and aorta were analyzed in the tissue sections using electronical microscope. In the second group of animals, the mean values of SBP – 153,6 ± 5,4 mmHg, DBP – 67,9 ± 2,8 mmHg, and HB – 394 ± 12/min were significantly increased in comparison with C group of animals (123,0 ± 5,2 mmHg (p<0,001), 55,6 ± 3,0 mmHg (p<0,05), and 361 ± 24/min (p<0,001), respectively). Such alterations in cardiovascular parameters correlated with marked increase in DOCA-salt group blood plasma levels of ET-1, IL-1, TNF-alpha, and NF-kB vs. C animals. S/V treatment significantly reduced SBP, DBP, and HB (-30,2 ± 4 mmHg, (p<0,001), -11,4 ± 2,2 mmHg (p<0,05), and -33 ± 12/min, p<0,002, respectively) in hypertensive rats (HR) associated with marked decrease in blood levels of ET-1 (56,9%, p<0,01), IL-1 (67,3%, p<0,001), TNF-alpha (75,1%, p<0,001), and NF-kB (59,2%, p<0,001), with significant improvement of morphological changes of the heart in this group of animals. It is suggested that S/V in hypertensive state exerts a positive effect on hemodynamic parameters, providing reverse action on heart remodeling and can be considered as a valuable drug for the AH treatment and prevention of structural changes in target organs.

Read More Background: N terminal pro B type Natriuretic Peptide (NT-proBNP) is an inactive pro-hormone that is used as a biomarker to evaluate our heart’s condition. It is secreted due to stretching of venctricles. Increment of NT-proBNP levels usually signifies worse cardiac condition. NT-proBNP has been also used to assess the other indirect conditions that have an impact on cardiomyocytes such as sepsis and dehydration.Objective: NT-proBNP is one of the functional cardiac markers that could be used as one of the markers in critical condition patients to measure cerebral perfusion and venous congestion. The better and the sooner understanding about NT-proBNP could help intensivists for relating with sepsis condition, resuscitation, and volume overload and prevent the multi organ failure.Methods: 30 unconscious intensive care unit (ICU) patients with various sources of infection and sepsis as a working diagnosis from q-SOFA criteria were selected. All of the patients were intubated with mechanical ventilation and oxygenation. The blood sample was taken from the jugular vein to measure NT-proBNP, haemoglobin, procalcitonin and frontal rSO2 are measured by using Near-Infrared Spectroscopy. The collected data include central venous pressure and mean arterial pressure.Results: Significant correlation between NT-proBNP and other markers such as, haemoglobin (p=0.001), right rSO2 (p=0.001), left rSO2 (p=0.001), and mean arterial pressure (p=0.001). NT-proBNP result also has a correlation with procalcitonin (p=0.011) and central venous pressure (p=0.011).Conclusion: NT-proBNP has a correlation with the other markers, such as procalcitonin, as one of the sepsis markers, haemoglobin, mean arterial pressure, and central venous pressure as components of delivery oxygen, and rSO2 as the recent brain oxygenation parameter that has been used. NT-proBNP definitely could be developed in the future as a useful marker in critical condition patients to support the primary organ sepsis patients in the intensive care unit.
