Read More Background: Postoperative Nausea and Vomiting (PONV) is defined as any nausea, retching or vomiting occurring during the first 24-48 hours after surgery with a reported incidence of 30% in all post-surgical patients and up to 80% in high risk groups especially in laparoscopic surgeries under general anaesthesia. Many drugs have been tried and tested for abolishing PONV. We evaluated the efficacy of low dose ketamine in decreasing the incidence and severity of PONV following elective laparoscopic surgeries under general anaesthesia.Method: 40 patients belonging to American Society of Anaesthesiologists Physical status I who were scheduled for elective laparoscopic surgeries under general anaesthesia were recruited and randomized into two groups Group A and Group B. Standard protocol for general anaesthesia were followed for all patients. Patients in Group A received Intravenous ketamine of 0.2mg/kg and Patients in Group B received the saline control half an hour before extubation. The incidence and severity of PONV, number of rescue drug administration, and side effects were recorded for first 24 postoperative hours in different time periods.Results: Demographic profile were comparable among the groups. The incidence and severity of postoperative nausea were significantly higher in Group B when compared to Group A for the first 6 postoperative hours (p<0.05) and thereafter, it was comparable between the groups. The incidence of postoperative vomiting was significantly higher in Group B when compared to Group A for the first 2 postoperative hours (p<0.04), and then it was comparable between the groups. The number of rescue medications were significantly higher in Group B when compared with Group A. No complications were noted.Conclusion: Low dose intravenous ketamine (0.2 mg/kg) administered as a single bolus reduced the incidence and severity of postoperative nausea and vomiting in adults undergoing laparoscopic surgeries under general anaesthesia without any complications.

Read More Females of childbearing age often suffer from polycystic ovary syndrome (PCOS), a hormonal condition. Symptoms including insulin resistance, high androgen levels, irregular menstrual (estrous) cycles, and polycystic ovaries are linked to this condition. The study's objective is to determine the therapeutic effect of Tinospora cordifolia stemextract and Alkaloid Rich Fraction against continuously Light Induced Polycystic Ovary Syndrome in Sprague Dawley (SD) female rats. Animals were grouped into six groups where the control group received only distilled water and the standard group received Clomiphene citrate 20 mg/Kg. The test groups received Tinospora cordifolia Stem Extract (200 mg/kg b.w and 400 mg/kg b.w) and Alkaloid Rich Fraction (10 mg/kg b.w and 20 mg/kg b.w) for the period of 4 weeks after PCOS induction. The results showed a significant reduction in elevated testosterone levels compared to that of the PCOS group (Light Induced Group (L/L 24 hrs Continuous), but follicle-stimulating hormone (FSH), insulin levels reported significant increase compared to that of the L/L PCOS group (Light Induced Group (L/L 24 hrs Continuous) and increased expression of INSR gene and restoration of melatonin levels. From the results, it was concluded that High doses of Tinospora cordifolia Stem Extract (400 mg/kg b.w) and Alkaloid Rich Fraction (20 mg/kg b.w) could be effective against Continuously Light Induced Polycystic Ovary Syndrome in Rodents it has due to decreased testosterone levels and increase insulin sensitivity by up-regulating INSR gene expression followed by restoration of melatonin levels. After treatment period a significant decrease in elevated LH levels, testosterone levels, and a significant increase in insulin and follicle-stimulating hormone (FSH) levels when compared to the L/L PCOS group (Light Induced Group (L/L 24 hrs Continuous), as well as increased INSR gene expression and restored melatonin levels.

Read More Background: In traditional Persian medicine, an herbal cold remedy containing Sugarcane, Black Myrobalan, and mastic is mentioned and it was being widely provided by grocery stores. With the advent of the COVID-19 pandemic, the increased desire for this traditional remedy has led to a debate in society. Huge parts of the Iranian population have started using this remedy for the treatment of COVID-19; while no study has evaluated its efficiency and possible side effects. Thus, we aimed to evaluate the effects of Sugarcane, Black Myrobalan, and Mastic‏ ‏herbal medications for COVID-19 patients.Methods: This was a double-blinded randomized clinical trial study conducted over three months from May 2020 to July 2020 in SARS-COV2 PCR positive patients admitted in the COVID-19 ward of Peymaniyeh Hospital in Jahrom, Iran. Patients with severe COVID-19 infection were not recruited. The intervention group received the routine COVID-19 treatment protocol and the herbal supplement received the combination of black myrobalan and mastic and sugarcane, twice a day (3 g of herbal supplement). Study groups were compared in terms of demographic variables, vital signs, and clinical and laboratory variables for safety and efficiency assessment.Results: Seventy-two patients with COVID-19, divided into intervention (n=37) and control (n = 35) groups were studied. Intervention and control groups had not any significant difference in terms of baseline characteristics. The time-to-event analysis revealed a statistically significant difference in 4 symptoms of cough, fever, dyspnea, and myalgia (P<0.05). There was no significant difference in averaged O2 vital signs between the two groups (P>0.05). The Control group in comparison to the intervention group had a significantly lower decrease in C-reactive protein during 7 days (P<0.05). Patients in the herbal supplement group were hospitalized for 4.12 days and patients in the control group were hospitalized for 8.37 days (P=0.001). ICU admission and death only happened in 3 (8.6%) patients of the control group.Conclusion: This study showed that the proposed herbal remedy could be applied as supplementation to conventional management of COVID-19 patients with mild disease, while more research is needed for clinical application of this remedy.

Read More The review article presents the results of studying plants growing on the territory of the State National Natural Park Tarbagatai, on the basis of which it will be possible to develop new medicines. In recent years, the popularity of herbal medicine, despite a great success in the creation of chemical drugs, has been growing. Interest in natural healing substances and drugs created on their basis is increasing due to both the unique properties of herbal remedies and rapidly developing research technologies in biology, medicine, and the production of drugs. The analysis and structuring of literature sources contain some information about herbal medicines, such as Corydalis ledebouriana Kar. et Kir., Malus sieversii, and Daphne altaica Pall. were carried out. They were grown in Asian countries, mainly in China and Kazakhstan. Many Corydalis species are rich in alkaloids that have a positive effect on the central nervous, digestive, cardiovascular, and pituitary-adrenal systems. Malus sieversii has a high antioxidant activity, it can inhibit cancer cell proliferation, reduce lipid oxidation, and lower cholesterol levels according to in vitro studies of animals, which may explain their roles in reducing the risk of chronic diseases. Daphne have a wide range of pharmacological activity, including anticancer, anti-inflammatory, anti-HIV, hypocholesterolemic, neurotrophic, anti-fertile, skin irritant, nematocidal activity, and pesticide activity. Phytopreparations have some advantages such as low toxicity with sufficiently high efficiency, a wide range of therapeutic action, complex organ protective effect, harmonizing effect on all organs and systems of the body, the minimum number of side effects, relative cheapness compared to synthetic drugs, and the possibility of cooking at home. Phytotherapy has some applications such as primary and secondary preventions of various diseases, health improvement, and rehabilitation of the general population under the influence of negative environmental factors, as a means of increasing the adaptive reserves of a healthy body in sports medicine.
