Read More Healthcare personnel involved in COVID-19 care have been negatively affected by the associated excess stress. This study aimed to investigate the relationship between moral distress and moral courage among operating room nurses due the hidden conditions of the effects of COVID during the outbreak of the coronavirus disease (COVID-19) crisis. This descriptive-correlation study was conducted on all operating room nurses working in educational and medical centers in Shiraz, Iran, 2022. The samples (n=141) were selected using a convenience sampling method. The required data were collected by questionnaires of demographic form, the Professional Sekerka Moral Courage and the Moral Distress Scale. The gathered data were analyzed in SPSS software through descriptive and analytical statistics. The mean scores of moral courage and moral distress only nurses who as Circuler Nurse were obtained at 61.67±7.79 was 47.4±18.9, respectively. It was revealed that there was a statistically significant relationship between the dimensions of moral courage and the dimensions of errors and inappropriate competencies and responsibilities of moral distress (P>0.05). The moral courage and moral distress of the nurses showed significant differences in terms of the type of employment, length of working experience, specialized field of surgery, and working ward, age, specialized field of surgery during the COVID-19 crisis. Moral agency and multiple values showed a statistically significant and direct relationship with total moral distress (R=0.20, P=0.01). Concerning the results of study, there can be stated that nurses were inclined to perform moral behaviors and an increase in their sense of responsibility during the COVID-19 crisis would lead to courageous behaviors and ultimately, a reduction in moral distress. Therefore, it is necessary that managers pay serious attention to the employment of training programs and improving the psychological knowledge of involved nurses and sharing nurses’ experiences of moral distress.

Read More Objective: COVID-19 has presented numerous epidemiological and clinical pictures from its beginning and much effort has been paid to detect the behavior of disease and its new types. Therefore, in this study, we aimed to compare the in-hospital survival time of Delta and Omicron variant patients admitted to the intensive care unit. Methods: This was a secondary data analysis of the QCOVICU data registry of 200 COVID-19 patients admitted to the ICU of Shahid Beheshti-Amir Al-Momenin Hospital of Qom City, in 2021. Likewise, time to event data, demographics, and baseline laboratory data was collected. Time of transfer to ICU, survivals, and possible predictors of hazards of death was compared within the variants of Omicron and delta.Results: Two hundred patients (62.98±19.94 years old, 94 females/106 males; 100 Delta and 100 Omicron variant) participated in this study. Fifty percent of the population had died. Cross-tabulation showed comparable death rates among variants of delta and omicron (50.5% vs. 51%; p=0.999). There was a statistically significant higher time to ICU admission in Delta variant victims than in Omicron variant victims. The mean survival time of delta variant patients was 21.52 days (95% CI: 17.96 – 25.09) which was statistically higher than the mean survival of omicron patients (17.15 days, 95% CI: 13.65-20.64, p=0.018). The mean survival time of delta variant patients was ‎statistically higher than omicron patients (21.52 vs. 17.15 days, ‎p=0.018). Gender, age (years), and lymphocyte count were significant predictors of mortality based on the Cox regression analysis (P>0.05). There was a 5.9 times higher risk of mortality in females compared with males’ gender after adjusting for other variables and a 5.6% increase in death risk with a 1-year increase in age, and a 31.8% decrease in death risk with a 1% lymphocyte percentage increase.Conclusion: Critically patients with Delta variant are getting ICU admitted later and withstand more days at ICU than Omicron patients. It seems that Omicron variant causes sudden deterioration of the patient's condition.

Read More Background: In traditional Persian medicine, an herbal cold remedy containing Sugarcane, Black Myrobalan, and mastic is mentioned and it was being widely provided by grocery stores. With the advent of the COVID-19 pandemic, the increased desire for this traditional remedy has led to a debate in society. Huge parts of the Iranian population have started using this remedy for the treatment of COVID-19; while no study has evaluated its efficiency and possible side effects. Thus, we aimed to evaluate the effects of Sugarcane, Black Myrobalan, and Mastic‏ ‏herbal medications for COVID-19 patients.Methods: This was a double-blinded randomized clinical trial study conducted over three months from May 2020 to July 2020 in SARS-COV2 PCR positive patients admitted in the COVID-19 ward of Peymaniyeh Hospital in Jahrom, Iran. Patients with severe COVID-19 infection were not recruited. The intervention group received the routine COVID-19 treatment protocol and the herbal supplement received the combination of black myrobalan and mastic and sugarcane, twice a day (3 g of herbal supplement). Study groups were compared in terms of demographic variables, vital signs, and clinical and laboratory variables for safety and efficiency assessment.Results: Seventy-two patients with COVID-19, divided into intervention (n=37) and control (n = 35) groups were studied. Intervention and control groups had not any significant difference in terms of baseline characteristics. The time-to-event analysis revealed a statistically significant difference in 4 symptoms of cough, fever, dyspnea, and myalgia (P<0.05). There was no significant difference in averaged O2 vital signs between the two groups (P>0.05). The Control group in comparison to the intervention group had a significantly lower decrease in C-reactive protein during 7 days (P<0.05). Patients in the herbal supplement group were hospitalized for 4.12 days and patients in the control group were hospitalized for 8.37 days (P=0.001). ICU admission and death only happened in 3 (8.6%) patients of the control group.Conclusion: This study showed that the proposed herbal remedy could be applied as supplementation to conventional management of COVID-19 patients with mild disease, while more research is needed for clinical application of this remedy.
