Read More Background: In comparison to the general adult female population, women afflicted by schizophrenia exhibit higher rates of unintended and undesired pregnancies. Maternal individuals grappling with severe mental health challenges face an increased susceptibility to postpartum psychosis, depression, and anxiety stemming from the presence of a child. Psychotic conditions often lead to the rejection of pregnancy, subsequently impinging upon maternal parenting capabilities. The predominant source of support for women with schizophrenia emanates from their familial networks.Case illustration: This paper presents the case of a 41-year-old adult female diagnosed with paranoid schizophrenia that was 40 weeks pregnant and admitted for medical care. The patient received treatment for restlessness and speech slurring. Comprehensive evaluations were conducted by healthcare professionals from the obstetrics and gynecology as well as psychiatry departments, leading to a decision to proceed with a cesarean section to terminate the pregnancy.Methods: Patients receive medical interventions and psychological assistance to stabilize their mental health conditions and avert relapses. Supportive psychotherapy and family psychoeducation, coupled with a collaborative approach involving discussions with obstetrician colleagues regarding childcare plans and supplementary psychiatric treatment, are implemented to enhance maternal well-being while attending to infant care.Conclusion: The obstetric team extensively deliberated the advantages and disadvantages of terminating the pregnancy, considering the provision of sustained care for both the patient and her unborn child. Given that the pregnancy termination was facilitated via cesarean section due to the patient's mental health concerns and the potential adverse effects of antipsychotic medication, the patient's family plays a pivotal role in providing assistance for infant care. Consequently, breastfeeding is contraindicated concerning the patient's mental health status and the potential consequences of antipsychotic treatment.

Read More Objective: The objective of this study is to investigate the occurrence and management of acute thrombotic microangiopathy, a severe complication associated with antiphospholipid syndrome (APS), particularly in the context of catastrophic APS (CAPS) during pregnancy. The study aims to enhance the understanding of CAPS in pregnancy, improve early diagnosis, and evaluate the effectiveness of different treatment modalities.Materials and methods: The study employed a case report methodology, focusing on a pregnant patient diagnosed with CAPS and experiencing acute thrombotic microangiopathy. The clinical presentation, laboratory findings, diagnostic procedures, and treatment interventions were documented and analysed. The treatment approach included the use of anticoagulants, glucocorticoids, and plasma exchange. The response to treatment, complications, and patient outcomes were assessed.Results: The study observed a rapid development of liver, central nervous system, and renal lesions within a week, indicating the severity of CAPS in the pregnant patient. Despite the pregnancy termination, the patient's clinical condition did not show improvement, suggesting a lack of response similar to observations in other cases. The patient received standard CAPS treatment, except for intravenous administration of immunoglobulins. Further details were reported regarding the treatment outcomes, laboratory parameters, and imaging findings.Conclusion: This study highlights the challenges associated with diagnosing and managing CAPS during pregnancy, specifically focusing on acute thrombotic microangiopathy. Early diagnosis, aggressive treatment with anticoagulants and glucocorticoids, and the use of plasma exchange or intravenous immunoglobulin are crucial in achieving positive outcomes. The study suggests the consideration of alternative interventions, such as immunosuppressive therapy or targeted biologic agents, in refractory cases of CAPS. Further research is needed to elucidate the optimal treatment strategies and long-term outcomes in pregnant patients with CAPS.

Read More Background: Gingivitis is an inflammation limited to marginal gingival tissues, which can be referred to as the mildest form of periodontal diseases, initial signs of inflammation which include: redness, bleeding, swelling, exudation, and to a lesser extent pain, which indicate the presence of gingivitis. Current research was proposed to detect the correlation between pregnancy status and oxidative stress (OS) levels among gingivitis pregnant women and to elucidate the role of pregnancy status in the pathogenicity of gingivitis.Methods: The current study was conducted in the Dental Center at Thi-Qar province (Iraq), and it included three study groups: 35 pregnant women with gingivitis, 35 non-pregnant women with gingivitis, and 30 non-pregnant women without gingivitis (the healthy control), with an age balance between the three study groups. Serological and salivary levels of malondialdehyde (MDA), total anti-oxidant capacity (TAC), protein carbonyl (Pc), protein, nitric oxide (NO), and ceruloplasmin (Crp) were biochemically determined.Results: The results of the tests showed that the concentrations of MDA, Pc, the total proteins (TP), NO, and Crp were significantly higher among pregnant and non-pregnant women with gingivitis compared with healthy controls in both serum and saliva, whereas the inverse findings were reported for TCA biomarker. The same results profiles were reported in comparison between pregnant women with gingivitis and non-pregnant women with gingivitis for serum biomarkers. For saliva biomarkers, the results revealed significant elevated levels of MDA, Pc, and NO among pregnant gingivitis women in comparison with non-pregnant gingivitis, whereas the other biomarkers revealed non-significant differences.Conclusions: The pregnancy status had an effect on the OS level which lead to a higher level of OS in both serum and saliva and the correlation between the pregnancy status and serum and/or saliva OS levels was positive which indicated the possible utility of its present lead to gingivitis development.
